Earnings Withholding for Court Ordered Debt (Garnishment)

We may recieve in the mail a letter from the franchise tax board.

One copy of the form will be sent to the employee. The other is for us to fill out and then return.

To set this up with ADP you will just have to contact their customer service and send them a PDF copy of the notice. They will set it up so that it will take out a specified amount per paycheck and then they will mail us a check that we can mail to the proper agency.

You must attach a copy of the form to the payment. You must also write your name, account number, and billing number as shown on the form on the payment as well. Copies of all forms can be kept here:
